form and send to the appropriate Foundation - once your fundraising
event has taken place and you have collected the funds and/or
once you have given the maximum number of hours you intend
to give to the charity this calendar year.
Please keep your own copy of the form in case
of queries.
Once you have sent in the form the cheque
goes directly to the charity. The most common reasons for
delay are:

Not fully completing the form (in particular
the registered charity number)

Not being specific on the form about
the fundraising activity and your involvement

Forms not being signed off by your Line
