There are separate claim forms for fundraising and volunteering.
Once your fundraising event has taken place and you have collected the funds and/or once you have given the maximum number of hours you intend to give to the charity this calendar year, Please complete appropriate Fundraising Matched Giving form or the Volunteering Matched Giving form.
We strongly advise that you read the
guidance notes
BEFORE completing any forms.
The form is now self checking when it is
completed, please print it off and get your line manager to
sign it and send to the appropriate Foundation.

of queries.
Once you have sent in the form the cheque
goes directly to the charity. The most common reasons for
delay are:

Not fully completing the form (in particular
the registered charity number)

Not being specific on the form about
the fundraising activity and your involvement

Forms not being signed off by your Line
